Reminder: Submit Form 1728a by January 20th

 

Brother Knights,

 Last week I sent you an email message regarding form 1728, Annual Survey of Fraternal Activity, and its importance for the Knights of Columbus in maintaining their tax-exempt status.  Councils are required to submit this report to the Supreme Council in January each year.  My message included a link to form 1728a Individual Member Worksheet (link). This form is for members to report the hours (calendar year 2025) they volunteered to perform charitable activities and/or community service outside of official council programs. The Council’s report will include all members’ time participating in council activities.  I ask you to complete and return the form to me by January 20th. 

 If you have not already completed your copy, please click on the link above, download the form, enter your volunteer hours, click on share and text or email it to me by January 20^th.  If you prefer, print the form, enter your data and return it to me at our meeting on the 20^th.
